Belgyógyászati Klinika (igazgató: Holló István dr.) Normál human plasma (NHP) infúzió hatását vizsgálták 37 thrombocytopeniás betegen. Öt heveny ITP-s betegben kilenc kezelési periódusban a 7—8. napra a thrombocyta-szám normális szintre növekedett. Tartós volt a remissió négy betegen. Húsz idzsít ITP-s és egy SLE-s betegen 36-ból 19 kezelési periódust követően növekedett csak jelentősen a keringő thrombocyta-szám. Legalább 20 ml/tests. kg NHP alkalmazásakor minden kezelési periódusban növekedett a thrombocyta-szám s a két végleges remisszió is e csoportban következett. Öt congestiv splenomegaliás thrombocytopeniás, továbbá hét aplastikus pancytopeniás betegen egyáltalán nem volt válasz, illetve mérsékelt, átmeneti thrombocytaszám-növekedés volt egy secundaeraplastikus thrombocytopeniás (melanomás) betegen. A hatásért felelős tényező nem azonos a thrombopoietinnel. Feltételeznek egy specifikus plasmafaktort, amely a megakaryocytaérés utolsó fázisára hatva segíti elő a thrombocyta-produkciót. De nem zárható ki az a lehetőség sem — hasonlóan az intravénásan nagy adagban alkalmazott IgG-hez, hogy a plasma-hatás is a macrophagok általi thrombocyta elimináció csökkentésében érvényesül. Hasznosnak vélik a NHP alkalmazását elsősorban heveny, steroid-rezisztens ITP kezelésében Eredmény várható az idült, vagy idült recidiváló, különböző kezelésre refrakter ITP eseteiben és más típusú immunthrombocytopeniák (SLE) alkalmával is. Célszerűnek látszik alkalmazása közvetlenül splenectómia előtt a súlyos thrombocytopenia megszüntetésére és a műtét hatásának potenciálására. Továbbá eredmény várható a splenectómia korai szakában (az első két-három héten) bekövetkező recidiva alkalmával. Az idiopathiás (immun-) thrombocytopenias purpura (ITP) kezelésében korábban több szerzőcsoport is alkalmazott friss normal human plasmát (NHP) (4, 23, 37, 37/b, 38, 44). Egy 1967-es összefoglaló munka szerint (1) a thrombocyta mentes plasma jelentős thrombocytosist eredményezett 46-ból 33 gyermekben. Tartós, komplett remissio 25 esetben volt. Több mint egy évtizedes szünet után ismét előtérbe került alkalmazása ITP-s gyer-Orvosi Hetilap 1984. 125. évfolyam, 23. szám Effect of normal human plasma (NHP) infusion in adults with idiopathic thrombocytopenic purpura (ITP). The effect of NHP infusion has been investigated on 37 patients with thrombocytopenia. In 5 patients with acute ITP (in 9 treatment-periods) the thrombocyte count increased within 2 days and reached normal levels on 7 to 9 days of onset of NHP infusion. Long-term remission was obtained in 4 of 5 cases. In 20 patients whith chronic or intermittent ITP and in 1 patient with SLE the circulating thrombocyte count increased significantly following 19 treatment periods out of the 36. But only in 2 patients developed long-term remission. There was no response at all in 5 patients with hypersplenic thrombocytopenia due to congestive splenomegaly and in 7 patients with aplastic pancytopenia, except 1 patient with secondery aplastic thrombocytopenia (due to melanoma) whose response was moderate and transient. It can be assumed reasonably that a specific plasma factor other than thrombopoietin or megakaryocyte colony stimulating factor exists promotes the platelet production by taking effect in the last steps of megakaryocyte maturation. The possibility, however can’t be exluded, that NHP diminishes the removal of antibody-coated platelets by the reticuloendothelial system by transient blockade of its function as it has been supposed in connection with the effect of high-dose intravenous gammaglobulin. These results indicate that the application of NHP could be considered for the treatment of acute ITP, refractory to steroid. Furthermore, result can be expected in chronic or recurrent ITP and other types of immunthrombocytopenia (SLE). NHP infusion seems to be advisable in ITP immediately before splenectomy (or other surgery) and in early relapse following splenectomy, in order to influence the life-threatening thrombocytopenia.
