placentalis AP gén terméke. A génnek jelenleg 3 allélja ismert és igen változatos (szervspecifikus) poszttranszlációs glikozidáción esik át az izoenzim, ami egyben nagyfokú polimorfizmusát is magyarázza (21). Széles körben vizsgálják az ún. placentális izoenzim tumormarker szerepének lehetőségét. Az eddig publikált adatok alapján úgy tűnik, hogy csak gonadális és gynecologiai tumorokban lehet hasznos marker. Extragonadális tumorokban csupán 10—24%-ban tapasztalható placentális AP megjelenése (16, 20, 24, 37, 39, 50, 56). A Aagao-izoenzim a „placental-like” AP gén terméke és elsősorban seminoma és ovarium tumorok (dysgerminoma, serosus cystadenocarcinoma) esetében bizonyult specifikus laboratóriumi markemek (21, 33, 56). A Kasahara-izoenzim valószínűleg heterodimer struktúrájú, azaz egy intestinális AP és egy placentális AP alegységből épül fel, de lehet az is, hogy a fetális intestinális gén derepressziójának terméke. A fenti 3 izoenzim közül bármelyik is jelenik meg a keringésben, eredetük közös: mégpedig a malignus transzformáció során a sejtekben fötális proteineket kódoló gének aktiválódnak, egyelőre általunk még ismeretlen mechanizmussal (21). Malignus tumoros betegekben ezek a „számfeletti” — azaz patológiás izoenzimek akkor is megjelenhetnek, ha a vérsavó AP aktivitása még nem tekinthető kórosnak. Az AP izoenzimek tumor markerként történő felhasználásának klinikai értéke jelentős. E tapasztalatot az a tény sem csökkenti, hogy e specifikus laboratóriumi paraméterek szenzitivitása alacsony. Gyakorlatilag tehát akár előrehaladott stádiumú malignus folyamat esetén is előfordulhat a referencia értékhatáron belüli AP aktivitás és fiziológiás AP-izoenzim konstelláció.
