A szerzők 25 primer aldosteronismus tüneteivel vizsgálatra került adenomas beteg (24 aldosteronoma, 1 DOComa) sebészi kezelésének eredményeiről számolnak be 3 hó—10 év megfigyelési idő távlatában. Betegeik közül 19 vált tünetmentessé, ötnél mérsékelt hypertonia jelentkezett, egynél hypermineralocorticismus tünetei nélkül (normokalaemiás, normoreninaemiás betegnél) progressiv hypertonia alakult ki. Az eredmények szemszögéből retrospective analizálják a betegek műtéti szelektálásának összetett praeoperatív diagnosztikai nehézségeit, az adenomák lateralizációjának lehetőségeit, a műtéti előkészítést, a sebészi beavatkozás módját, az intra- és postoperatív szövődményeket, az adenomák és a környező mellékvese, illetve a másik oldali mellékvese morphológiai adatait. Megállapításaik szerint a praeoperatív diagnosztika legbiztonságosabb módja a nyugalmi és a renin-angiotensin systema stimulációját követő renin-aktivitás, aldosteron és 18-OH-corticosteron plasma szintjének meghatározása. A lateralizácós módszerekben a mellékvese véna aldosteron/cortisol hányadosa jelzi legmegbízhatóbban az adenoma oldaliságát. A mellékvese adenoma eltávolításának transabdominális módját célravezetőnek tartják, nemcsak a kisszámú szövődmény miatt, hanem mert ilyen módon lehetőség van a nem adenomás mellékvese exploratiójára is. Felhívják a figyelmet az adenomát környező mellékvese szövet morphologiai vizsgálatának jelentőségére, amely a prognosis vonatkozásában meghatározó, mivel a soliter adenoma melletti micro-, illetve macronodularis hyperplasia jelenléte a hypertonia későbbi újrafellépését előre jelezve e betegek szoros ellenőrzését indokolja évek távlatában is. Retrospective evaluation of surgical treatment of primary aldosteronism on the basis of preoperative diagnosis and adrenal morphology. Authors analyse the effectiveness of surgical treatment of 25 patients hospitalized with the symptoms of primary aldosteronism (24 aldosteronoma, 1 DOC- oma) and observed 3 months — 10 years after operation. Nineteen patients became Symptome free, five remained moderately hypertensive, one (who were normokalemic and normoreninemic before operation) remained with progressive hypertension but without the symptoms of hypermineralocorticism. In the light of the results, a retrospective analysis was done considering the preoperative diagnostic difficulties by selecting the patients for operation, the possible lateralisation of adenomas, preoperative treatment, technique of surgical intervention, the intra- and postoperative complications, morphological characteristics of adenoma, the surrounding adrenal tissue and contralateral adrenals. It is concluded that the most reliable diagnostic tools are the determination of basal and specifically stimulated renin activity, the measurement of the plasma level of aldosterone and 18-OH-corticosterone. Among the methods for lateralisation, the aldosterone/cortisol ratio in the adrenal venous blood is the most indicative. The taransabdominal (midline) approach for removing the adenoma is considered to be the most appropriate, because the surgical complications are negligible and this way the contralateral adrenal can also be easely inspected. Authors point to the importance of morphological analysis of the tissue surrounding the solitaer adenoma, in which a micro- or macronodular hyperplasy might be predictive for a possible occurance of hypertension and therefor neccessiates a careful and long-term postoperative check-up of these patients.% Orvosi Hetilap 1985. 126. évfolyam, 35. szám 2139
